THE PATIENT PATHWAY

To ensure you receive the highest level of care and support we have developed the Patient Pathway Program. We are the only company to provide such a program. It is designed to help you every step of the way and give you the smoothest path possible on your journey to living a longer, healthier, happier and more fulfilling life.



Identification of sleeping disorder

You are a referred to an independent specialist Sleep Physician to identify whether or not you have Sleep Apnea.

If you think you may be at risk of Sleep Apnea you can contact your nearest Sleep Technician who can discuss your symptoms and with you help you take the first steps along our Patient Pathway Program. This involves getting a referral from your GP to undertake a sleep assessment with Healthy Sleep Solutions. You should discuss any concerns with your doctor.

Diagnosis & Testing

Our program allows you to undertake a sleep assessment in the comfort of your home. Your independent specialist Sleep Physician will assess your results and recommend the appropriate treatment if you are diagnosed with Sleep Apnea.

On the day of your sleep test you will meet with your Sleep Technician who will take a brief medical history, provide you with more information on Sleep Apnea and show you how to use the equipment that will test you for Sleep Apnea.

Your Sleep Technician will also book you in for another appointment to review the results of your sleep test.

Treatment of Sleep Apnea

It begins with a multi-week Treatment Introduction providing you all the personalised coaching and support to ensure the successful treatment of your Sleep Apnea.

If you are diagnosed with Sleep Apnea and your specialist Sleep Physician has recommended CPAP therapy, we will provide a multi-week treatment program.

During this time your Sleep Technician will meet and speak with you as often as you need to help with the equipment, answer any questions you may have and offer advice for getting the most out of your treatment.

The aim of this program is to ensure you have the device, mask and accessories that will result in the most comfortable and effective management of your Sleep Apnea.

The Sleep Technician will also provide you with an update on how the treatment is progressing by using data captured on your CPAP device. Your referring doctor will also be kept up to date on how your treatment is progressing and how well you are responding to it.

As part of our service, we will offer payment plans on your treatment device.

If the Sleep Physician has recommended a mouthguard (a Mandibular Advancement Splint) we can facilitate this treatment through our partners.

Ongoing support

Your Sleep Technician will always be there to offer support and advice.

Whether you are at the start of your treatment or have been on treatment for many years, your Sleep Technician is available to help you with all of your Sleep Apnea needs.

They are there to co-ordinate all aspects of your care and long-term CPAP treatment. This includes:

  • Continuing advice and support in getting the most out of your treatment
  • Equipment servicing including parts and upgrades
  • Treatment reviews and adjustments
  • CPAP device downloads to ensure your treatment is still effective
  • Advice on travelling with your CPAP device
  • Ongoing annual CPAP treatment reviews with your Sleep Technician

Your Sleep Technician will also help with any equipment issues you may face including organising loan equipment if your machine needs to be sent in for repair and showing you the newest technology available for Sleep Apnea treatment.
